Team Overview
The high rate of hypertension among this population has many underlying causes including environment, genetics, and individual lifestyle behaviors. The project will focus on self-empowerment, individual accountability, and education through health literacy and peer-to-peer support to reduce hypertension rates. The project will have two interventions:
- Creation and distribution of DVDs that use personal stories to promote adherence to prescribed hypertension medication instructions
- Completion of a health literacy screening for patients and training for physicians to modify patient communications based on screenings’ results
